Eligible customers may be able to receive reimbursement for MilkHaven purchases using HSA or FSA funds.

How reimbursement works
1. Complete the consultation
Before purchasing, complete a brief online consultation through Flex. If eligible, you’ll receive a Letter of Medical Necessity from a licensed provider.
2. Purchase MilkHaven
Complete your MilkHaven purchase using a standard payment method. Do not use your HSA or FSA card directly at checkout.
3. Submit for reimbursement
Send your Letter of Medical Necessity and MilkHaven receipt to your HSA or FSA administrator for reimbursement.
Before you begin
The Flex consultation costs $15 and may itself be HSA/FSA eligible.
Your Letter of Medical Necessity is valid for one year and may apply to eligible future purchases made during that period.
Complete the Flex consultation before purchasing MilkHaven. Only purchases made on the same day as, or after, the consultation may qualify for reimbursement.
Eligibility and reimbursement are determined by the licensed provider and your individual HSA or FSA administrator.
